The Banner|Aetna Care Advocate Team outreaches, at no extra cost to eligible members, to those members most in need of extra coordinated care to help them reach their health goals. Whenever possible, they meet face-to-face with members to help improve communication and coordination. Engaging with the Care Advocate Team is your first step to better health.

Our Care Advocate Team consists of nurses, social workers, diabetes health educators and pharmacists who will help coordinate your care and put your needs first.* They work with your primary care providers to help identify any barriers to your care goals and find ways to address them. Your team will help you make sure that your care works for you and will provide you with information regarding effective and affordable treatments. We are committed to helping you get the care you need.
*Members of the Care Advocate Team do not provide medical care to members; instead, they help members get the care they need.
