Juan's Story

In 2017, Banner|Aetna created a new care model that brings a multidisciplinary team into local Arizona neighborhoods.

The team consists of nurses, a pharmacist, social workers, a dietician and certified diabetes educator. Together, they provide high-touch care management that addresses members’ medical and social needs

So far, the team has served hundreds if members..

Juan is just one of those members.

My health issue, it can't be fixed overnight, but it can be fixed. My concerns were, I'm not going down the right path. My name is Juan Javier, and I've been with Banner Aetna for over a year now. When I first started dealing with my Banner health team I was skeptical. With this team I have now, they seem to put their arm around me. I have a Pharmacist, Dietician, I have my Case Manager, Wesley. She showed up at my job just to check on me too, so that's nice. Damion, who is the Dietician, showed me what I should be looking for as far as food and nutrition, what to have and what not to have, and the portions that I should be having. I truly enjoy this team. They have really changed it for me, my life has been better, my health has been better, I've lost some weight, and I think everybody should have this type of healthcare. With this coaching staff that I got, I'm really liking it. My concerns are really not there anymore. They know my story, and I can call them at any time, and that's what's the best thing, I do have that support. It helps me stay focused on my health, whereas before, once you leave the door you're on your own. Here, the doors never close.
